Reminder for 6th Grade Parents: Please mail or fax a copy of your child's most recent physical and documentation that your child has had a Tetanus Booster. (Most often documented as Tdap or TD) The booster must have been given within the last 5 years. This is a state mandate for entry to 7th grade. Please send all information to the nurses as soon as possible. The Nurse's Office Fax Line is 978-762-8332.
