Reminder for 6th Grade Parents: Please mail or fax a copy of your child's most recent physical and documentation that your child has had a Tetanus Booster (most often documented as Tdap), 2 doses of the Chicken Pox (Varicella) vaccine (or disease) and 2 MMR vaccines (measles mumps, rubella). The Tetanus (Tdap) booster must have been given within the last 5 years. This is a state mandate for entry to 7th grade. Please send all information to the nurses as soon as possible.
